People had expressed extreme caution with installing Flex GIF Animator v8.54 because of the registration procedure. What is the basis for that caution? What happened? Or in other words, what is your nightmare?

A Manual procedure - requiring sending an email, someone opening it - getting a number and generating a code and replying might work when they have to send out perhaps 500 codes in a day. For GOTD and the number of people who expect a code immediately - this does not work.
Then (apparently) when the code is received - the user is given a link to a new program and told to uninstall the downloaded giveaway, and download and install a new version of the program from the developers website, and and key in a new code to activate the second download.... huh?!?
(Oh incidentally, the original code can be "copied" using the provided "copy" button - but it drops a few digits from the code, making it invalid - so if you rely on the mechanism they provide to copy and paste the original code into an email - you are already on your way to "failure town").
In the past Most giveaways (where the developer handles the activation instead of GOTD) have had problems with the developers email server crashing, bandwidth limits being exceeded, hosting spam protection cutting in because too many emails are being sent out by the developer, wrong codes being sent by users requiring checking, a reply and a resend.
Activate.exe is the best way to get a giveaway running - it has been tested and allows users to unlock the software almost instantly and isn't really affected during peak periods (online automatic activation can handle a much higher load than someone manually reading emails from users and replying).
And then there is the issue of users who participate with giveaways because there is no requirement to provide an email address. In the past there have been email addresses "leaked" or "taken" from the developers website and users (including myself) were spammed on those email addresses.
Trying to be "unique" or making it easier on the developer by leaving their "manual" or email based activation - only creates different problems for the developer - and pisses off users who feel like they are being made to jump through hoops when they are used to normally being able to activate giveaways in a fairly hassle free way.
And it is coming up to 3 hours waiting for a code to be able to fully use this software. Normally it takes 10, perhaps 15 seconds.
